About the role

Responsibilities

  • Evaluate and manage acute and chronic wounds in mobile and facility-based settings.
  • Create and update treatment plans using evidence-based wound care practices.
  • Coordinate with facility staff, home health teams, caregivers, and supervising clinical leadership.
  • Document visits clearly and communicate changes in patient status promptly.

Ideal candidate

  • Licensed Nurse Practitioner with strong clinical judgment and patient communication skills.
  • Wound care experience preferred; motivated clinicians with strong fundamentals are encouraged to apply.
  • Comfortable working independently while staying connected to a supportive clinical team.
  • Reliable transportation for mobile patient visits in the assigned market.
